Set up #1:

Option 1:
List the pricing in the description and build the prices on the modifier. The item must have 0 price and the modifier should be required (Minimum 1 and Maximum 1)

Option 2:
No pricing in the description. Pricing is only shown on the modifier. The item must have 0 price and the modifier should be required (Minimum 1 and Maximum 1)

(Note: The modifier I created does not have pricing but when you build the modifier like this each size should have the pricing like the 5" pizza option)
